This is first shown when Muzan fails to heal an illness or injury he has suffered. Muzan finally killed the doctor and did not regret his actions at all. This selfishness also made Muzan create rules to prohibit his demons from committing treason or rebellion while preventing anything that could endanger his life. So, despite how well-known Muzan was for his power and existence as the first demon, he was actually very afraid of death.
