Muzan’s fear of death stems from his horrific experiences as a human. Muzan’s life was often on the brink, including when he was about to be born. This is what then gives rise to the nature where Muzan always tries to protect himself without caring about the safety of others by forming his demon army.
This is first shown when Muzan fails to heal an illness or injury he has suffered. Muzan finally killed the doctor and did not regret his actions at all. The curse was then created in order to maintain its existence on earth. This selfishness also made Muzan create rules to prohibit his demons from committing treason or rebellion while preventing anything that could endanger his life. So, despite how well-known Muzan was for his power and existence as the first demon, he was actually very afraid of death.
